O.M.R. application forms with instruction can be purchased for Rs.110/- (Application fee - Rs.100/- + Postal Charges Rs.10/-) for General candidates, Other backward caste candidates of Uttarakhand and Dependent of freedom fighter candidates of Uttarakhand and Rs.70/- (Application fee Rs.60/- + Postal Charges Rs.10/-) for Scheduled caste candidates of Uttarakhand and Scheduled Tribe candidates of Uttarakhand in cash through the post offices mentioned below from June 01, 2009 to June 30, 2009.

Candidates must obtain instructions alongwith the O.M.R. application form in order to fill it up.

Instructions for filling the O.M.R. Application Form

1. Candidates must go through the advertisement thoroughly and they should apply only after they are satisfied that they fulfill all the conditions of eligibility. in case of any deviation from the conditions of this advertisement, the application shall be rejected.
2. Candidates must mention the subject / cadre at the appropriate column of O.M.R. application form for the post of lecturer male / female cadre, otherwise their application form shall be summarily rejected.
3. Candidates affix postal stamp Rs.30/- on window envelope provided with the O.M.R. application form. Candidates must paste one self attested passport size photograph on the O.M.R. application form at the space provided for this purpose, otherwise their application form shall rejected
4. Candidates desirous of getting the benefit of one or more reservation must mention their category / categories (one or more as the case may be) in the relevant column of application form otherwise they shall be deemed to be general candidates and they shall not bo given the benefit of reservation.
5. Candidates must retain a photocopy of their O.M.R application form, and in all communication to the commission they must mention the O.M.R. application form number Application forms completed in all respect must reach the "Secretary, Uttarakhand Public Service Commission, Gurukul-Kangri, Haridwar - 249404" either by registered post / Speed post or by hand on or before 6:00 P.M June 30,2009 Application forms through any other means shall not be accepted.
